1. What is the procedure of applying for the Income Certificate?
Application duly filled in the annexure III with a court
fee stamp worth rupees five should be submitted to the Village
officer. The documents of proof required are that of property,
salary certificate or ration card. Village officer after
making necessary inquiry will issue the certificate.

What is the procedure for applying for the Nativity Certificate?
There is no specific application form for applying to this
certificate. Application duly written in the white paper
with a court fee stamp of rupees five should be addressed
to the Village Officer. Documents required are that of property,
Salary certificate (if employed) or Ration card.
Nativity certificates, which are to be given before the
defense personnel, should be collected from the Additional
District Magistrate (Deputy Collector) after giving the
application addressed to him.
For the people those who are residing outside Kerala should
submit the residential certificate along with this application
SC/ST
communities are exempted from paying the fee of five rupee
court fee stamp.

Application duly filled should be submitted to the Tahasildar,
with a stamp paper worth 50 rupees along with the documents
of property, or the kudikidappu certificate
of 12years. Tahasildar will be issuing the certificate based
on the report of the Village Officer or Revenue Inspector.

Application
duly written in a white paper with a court fee stamp of
rupees five should be submitted to the village officer.
Based on the report of the Village Officer, the certificates
will be issued by the Tahasildar or by other higher authorities.

If failed in registering death or birth at the right time.
Revenue Divisional Officer can consent this registration

be given to the Revenue Divisional Officer.
16.
What is the procedure of applying for the Learners
License?
Applicant should be of 18 years of age. But if the vehicle
is below 50 CC, the minimum age required is 16 years. If
the vehicle is for transportation purpose, the minimum age
required is 20 years.
Application duly filled in Form A along with the documents
of proof of residence such as SSLC Book, ration card, telephone
bill, LIC policy or electoral roll, should be submitted
to the Motor Vehicle Office of the concerned area. Other
important documents required are, Medical certificate in
Form 1 A (Not required for driving private vehicle) two
copies of Form 3, three passport size photographs, and fee
receipts.
There will be a test, if passed in the test; the applicant
will be given a learners license for 6 months. By
paying the fees the learners license can be renewed.
If failed in the test, by paying another fee can appear
for the new test after seven days.
17.
What is the procedure for applying for a driving license?
If obtained a learners license, a person can apply
for the driving test after 30 days. Documents required are,
an application duly filled in Form A, Learners license,
Fee receipts and three passport size photographs. If the
license required is for a transport vehicle Form 5 obtained
from a recognized driving school is also required.
If passed in the test the license will be given in short
span of time. If failed, by paying another fee again the
person can appear for the test but only after 7 days. But
if failed three times, he can only appear for the next test
only after 60 days.
A person who is having an international license or a Military
license is exempted from appearing the test.
18.
What is the procedure of applying for adding a new sort
of vehicle to the License?
If the vehicle is a transport one, the applicant should
have completed minimum 20 years of age, and completed minimum
one year after obtained an LMV driving license. But there
is no such condition if the vehicle is an autorikshaw. The
documents of proof required are, valid learners license,
application in Form 2, receipts of fees, and present license.
If passed in the test the license will be given in a short
span of time, by producing the result of the test

Application in Form LTA, attested conduct certificate by
a gazetted officer, MP, MLA, Panchayat President, Municipal
Chairman or Mayor. Medical Certificate in Form 1A, two attested
photographs, present License are also required. After the
test, License and Badge can be obtained by producing the
result of the test.

are required.

be produced before the Licensing Authority.

License?

copy of passport, attested copy of Visa, present driving
license, copy of air tickets are required. After the test
the License will be given for a period of one year, which
cannot be renewed.
25.
What is the procedure of the temporary registration of a
Motor vehicle?
Temporary registration is available for a period of one
month. This registration is given from the place of purchase,
for taking the vehicle to the place of registration. If
the Vehicle is a chassis, registration will be available
for a maximum period of 4 months.

What is the procedure of registering a new vehicle?
Application for registration should carry the place of permanent
residence or the place of working, or doing business. The
address, which is written in Form 21, is the one, which
is considering for the registration. The documents of proof
required are application in Form 20(If there is any hire
purchase agreement two copies are required), Sale certificate
in Form 21,certificate in Form 22, insurance certificate,
temporary registration, if any, Form 22 A (if the body is
built on a chassis) proof of residence and age, the pencil
print of chassis number, if the Vehicle is an imported one,
the customs clearance certificate and receipts of fees.
The Registration certificate will be given in a short span
of time.

What is the procedure of registering an outside vehicle
in the state?
Application duly written in Form 27, No Objection Certificate
in Form 28 obtained from the original registering authority,
registration certificate, insurance certificate, proof of
residence, pollution control certificate are also required.
If to change the ownership, two copies of Form 29 and Form
30, if to change the residence; application in Form 33,
receipts of fees should also be submitted along with the
application.
28.
What is the procedure of applying for No Objection Certificate
to outside States?
Three copies of Application duly filled in Form 28, (Court
fee stamp of worth Rs 5 should be attached in one copy)
along with the documents such as Address of the registering
authority, the pencil mark of the chassis number, attested
copies of registration certificate, pollution control certificate,
insurance certificate, receipts of taxes paid should also
be given along with the application. If the vehicle is using
for the transportation purpose, the permit should be surrendered.
29.
What is the procedure of changing the vehicle to the legal
heirs name after the death of the owner?
If the registered owner of a vehicle is dead that should
be informed to the authority immediately. The vehicle of
the deceased can be used for 3 months without changing the
name. For transferring the vehicle to the name of the legal
heir, application should be given in Form 31 with the documents
such as death certificate, certificate proving the right,
insurance certificate, registration certificate, pollution
control certificate, proof of residence and age and receipts
of fee should also be submitted along with the application.

Application duly written in Form 25, the documents such
as insurance certificate, pollution control certificate,
registration certificate, receipts of fees, should also
submitted along with the application. This application for
renewal should be submitted 30 days before expiring the
registration.

objection certificates from the company, receipts of tax

How to open a savings bank account ?
Savings Bank Accounts (SB A/Cs) are intended to help customers
by encouraging the habit of savings. It helps the customers
keep their surplus funds with the bank and earn interest
while providing the flexibility for withdrawals. SB A/Cs
can be opened by an eligible individual in single name or
jointly with others and by certain organizations/agencies
approved by The Reserve Bank of India. The banks will identify
customer on the basis of documents provided by the customer
such as
(a) Proof of identity and
(b) Proof of address.
The customer has to submit the concerned application form
along with Photographs.
(a) Proof of identity (any of the following with authenticated
photographs there on):
Voter ID card/Passport/PAN Card/ Govt./Defense ID card/ID
cards of reputed employers/Driving License
(b) Proof of current address (any of the following)
Credit Card Statement/Salary slip/Income/Wealth Tax Assessment

In case of joint accounts, applicants who are not closely
related to each other would be required to establish their
identity and address independently.
The minimum balance required to be maintained in a Savings
Bank Account w.e.f.01.01.2001 is as under:

Service charges are applicable for non-maintenance of the
above mentioned minimum balance in account on the basis
of average minimum balance maintained during each quarter.
No operation (withdrawals/drawings) for the last 12 and
24 months render a Savings Bank Account ‘dormant’
and “inoperative” respectively.

How to get new electric connection?
Application form for electric connection is available
in the section offices and "Friends" counter on
payment of Rs.25/-. Application can also be downloaded from
the website, in which case, the application fee has to be
remitted in the section office while submitting application
for new connection. The duly filled application along with
completion certificate, duly signed by the licensed wireman,
Supervisor and contractor has to be submitted to the Assistant
Engineer. Service connection estimate will be prepared and
intimation given to remit cash deposit and applicable service
connection charges. Cash deposit and service connection
charges depending on the connected load will have to be
paid. On remitting cash deposit and service connection charges
if any, consumer number will be allotted. To get connection
under OYEC (Own Your Electrical Connection) category, OYEC
charges also has to be remitted simultaneously. The waiting
time may vary according to the nature of connection. Service
will be effected in the fastest stream and priority will
be given to the following categories.
